New Jersey uses the Underground Storage Tank Facility Certification Questionnaire for facility registration/certification information. Among other fields, it asks for Class A and Class B operator contact information.
Why it matters
- DEP needs to know who the trained A/B operators are
- Missing or wrong designations create compliance risk
- Registration problems can cascade into operational pain

When you hire us for program management, we help designate our licensed Class A/B operator on that questionnaire path and keep the designation consistent with your agreement. We also make sure Class C training records exist.
Always use the current official forms from NJDEP. Links and form numbers change; we’ll point you to live versions when we work together.
Owner tip
When managers leave, update designations promptly. A certificate in a drawer doesn’t help if the questionnaire still lists someone who quit two years ago.
