UST Facility Certification Questionnaire — what owners should know

This is the form where your Class A and Class B operators become “real” to NJDEP.

New Jersey uses the Underground Storage Tank Facility Certification Questionnaire for facility registration/certification information. Among other fields, it asks for Class A and Class B operator contact information.

Why it matters

  • DEP needs to know who the trained A/B operators are
  • Missing or wrong designations create compliance risk
  • Registration problems can cascade into operational pain
